Position Purpose: Develop and facilitate complex care management activities for members with primarily physical needs, creating personalized care plans and providing education for members and families.

Key Responsibilities include:

  • Assessing member needs, barriers, and social determinants of health, and developing appropriate care plans.
  • Collaborating with providers and community resources to meet members' unmet needs.
  • Coordinating care between members, families, caregivers, and providers.
  • Monitoring member progress and updating care plans as needed.
  • Providing resource support and education to members and families.
  • Ensuring compliance with regulatory requirements.
  • Performing outreach via phone, digital means, or home visits.
  • Documenting all activities accurately.
  • Other duties as assigned.

Qualifications: Nursing degree from an accredited institution or a Bachelor's in Nursing with 2-4 years of experience. RN licensure in Illinois or a compact state is required.

Pay range: $55,100 - $99,000 annually. Benefits include health insurance, 401K, tuition reimbursement, paid time off, and flexible work arrangements.
