Position Details

This position is remote/work from home with 25% travel in the Cook County, Illinois area to support our YouthCare/Foster Care program. Applicants must reside in one of the specified Illinois ZIP codes.

Position Purpose

Develops, assesses, and facilitates complex care management activities for members with primarily physical needs to provide high-quality, cost-effective healthcare outcomes, including personalized care plans and education for members and their families.

Responsibilities
  1. Evaluate member needs, barriers to care, and social determinants of health, focusing on the member’s priorities, and recommend or facilitate the best care plan.
  2. Develop ongoing care/service plans, collaborating with providers and community resources to address unmet needs.
  3. Identify problems/barriers and provide appropriate care management interventions.
  4. Coordinate between members, families/caregivers, and care teams to ensure appropriate, person-centered care.
  5. Provide follow-up and monitor member status, updating care plans as needed.
  6. Support members with local resources for various services based on assessments.
  7. Facilitate care management and ensure timely access to needed services.
  8. Perform outreach via phone, digital means, or home visits to assess needs.
  9. Document and maintain all member information and activities in compliance with regulations.
  10. Educate members and families on disease processes, care gaps, and healthcare options.
  11. Provide feedback for quality improvement.
  12. Perform other duties as assigned.
Qualifications
  • Degree from an accredited nursing school or Bachelor’s in Nursing with 2-4 years of related experience.
  • RN license in Illinois or a compact state.
Compensation and Benefits

Pay range: $55,100 - $99,000 annually. Benefits include health insurance, 401K, stock purchase plans, tuition reimbursement, paid time off, and flexible work arrangements.
