Vigilance Health invites a qualified Quality Improvement Project Manager to bridge Vigilance with its ACO and population health partners. You will lead stratification, measure performance, and drive improvement projects that close gaps in care and boost quality scores.
You will work closely with partner QI teams and Vigilance leadership, translating data insights into actionable interventions and expanding patient panels across our network.
Quality Improvement Project Manager — ACO & Population Health RN
Remote | Full-Time
You Speak Fluent Data — and Fluent Relationship.
You know ACO quality measures cold. Hand you a stratified patient panel and you don't just see numbers — you see the story behind them: where the gaps are, who's falling through the cracks, and what it will take to close them. You've built trust with partner teams by showing up as a problem-solver, not just a reporter of scores.
Now imagine doing that as the connective tissue between Vigilance and every partner we serve — backed by a purpose-built population health platform and a company whose mission depends on the work you do. That's this role.
Vigilance Health partners with FQHCs, ACOs, IPAs, and payers to deliver Population Health and Care Management services that actually move the needle. We run Care Management, APCM, Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs — and we measure success the way our partners do: quality metrics, patient engagement, and outcomes that show up in data.
Our purpose is simple, and we mean it: Change People's Lives — no matter their circumstances.
As our Quality Improvement Project Manager for ACO & Population Health, you'll be the bridge between Vigilance and its partners — the subject matter expert partners turn to for population health strategy, ACO quality measures, and quality improvement execution. You'll use ACG to stratify our patient populations, identify opportunities to improve outcomes and scores, and turn those opportunities into action alongside partner QI teams and our internal clinical leadership.
You'll also be Vigilance's ACG subject matter expert — the person who can make comprehensive patient stratification make sense to a room full of stakeholders and who tells the success stories that prove its value. Beyond service excellence, you'll actively look for ways to grow the relationship: expanding the patient panels we manage and helping partners see new services worth launching.
You'll build and maintain relationships with every partner QI team and Vigilance-facing representative, becoming the face of Vigilance they turn to first. Trust is the foundation everything else gets built on.
You’ll go deep into ACG and the Vigilance Care Portal to understand the data, technology, and reporting behind our patient populations — for individual partners and Vigilance as a whole — and use that knowledge to find opportunities others miss.
You’ll generate and maintain baseline reports on ACO quality measures for every partner and for Vigilance overall, giving everyone a clear, current view of program effectiveness and where the opportunities are.
You’ll develop data-driven strategies for partners and targeted populations, then work with the Clinical Director to translate them into real changes — to workflows, trainings, and interventions that move outcomes and scores.
You’ll keep a continuous read on partner populations, emerging challenges, and shifting regulations, using what you learn to identify new services worth launching, drive agreement renewals, and expand the patient panels Vigilance serves.
You're charismatic and supremely confident in your population health and quality improvement expertise — the kind of person who can walk into a room with a CMO and hold your own. You're extroverted, empathetic, and outgoing, and you genuinely enjoy building relationships as much as you enjoy digging through data to find solutions. You're a highly effective communicator who loves technology and is usually one of the first to adopt it, and you think on your feet when a conversation doesn't go the way you planned.
If you've been looking for a role where your population health expertise and your relationship-building instincts both matter equally — this is it.
Vigilance Health is an equal opportunity employer committed to building a diverse, inclusive team.
