• Master's degree in Behavioral Health or Social Work, or a Nursing degree.
• 2–4 years of related experience.
• Relevant licensure as LCSW/LMSW/LMFT/LMHC/LPC or RN state licensure.

Responsibilities

• Evaluate member needs via phone or in-home visits and develop care plans.
• Conduct telephonic, digital, home, or site visits to assess needs.
• Develop ongoing care plans for high-acuity members and coordinate providers.
• Coordinate between members, families, community resources, and care teams.
• Monitor plans and member status for treatment changes and provide recommendations.
• Facilitate care coordination and ensure timely access to services.
• Document and maintain member information for compliance.
• Educate members and families on procedures and benefits.
• Provide feedback to leadership on care delivery improvements.

About This Role

The Care Manager (BH) develops, assesses, and facilitates complex care management activities primarily for members with mental and behavioral health needs, aiming to provide high-quality, cost-effective healthcare outcomes.

Highlights

• Schedule: Monday – Friday, Full-time
• Residency: Must live in Reno, Stafford, Rice, or Barton counties
• Travel: Up to 75% local travel

What You'll Do

• Evaluate member needs via phone or in-home visits and recommend/facilitate care plans.
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs.
• Develop ongoing care plans for members with high acuity and identify necessary providers and resources.
• Coordinate between members, families, community resources, and care provider teams.
• Monitor care plans/service plans and member status for changes in treatment and provide recommendations.
• Facilitate care coordination and collaborate with providers to ensure timely access to services.
• Collect, document, and maintain member information and care management activities for compliance.
• Provide education to members and families on procedures, treatment options, and healthcare benefits.
• Provide feedback to leadership on opportunities to improve care and quality delivery.
• Perform other duties as assigned and comply with all policies and standards.

Requirements

Education

• Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing

Experience

• 2 – 4 years of related experience

Licenses & Certifications

One of the following licenses required:

• LCSW (Licensed Clinical Social Worker) - required, OR
• LMSW (Licensed Master Social Worker) - required, OR
• LMFT (Licensed Marriage and Family Therapist) - required, OR
• LMHC (Licensed Mental Health Counselor) - required, OR
• LPC (Licensed Professional Counselor) - required, OR
• RN (Registered Nurse) - State Licensure required
