Job summary

Centene is seeking a Care Manager to develop, assess, and facilitate complex care management for members with mental and behavioral health needs. The role emphasizes high-quality, cost-effective outcomes and close collaboration with care providers and families.

You will evaluate needs, coordinate with providers, and maintain compliant records while educating members on healthcare options. This is a full-time, day-shift position with remote work supported in Georgia.

Qualifications

• Master's degree required in Behavioral Health or Social Work or Nursing.
• 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional or RN per state contract requirements.

Responsibilities

• Evaluate member needs via phone or in-home visits and propose care plans.
• Perform telephonic, digital, home, and other site visits to assess needs.
• Develop ongoing care plans for high-acuity members and coordinate providers.
• Coordinate between members, families, community resources, and care teams.
• Monitor plans for changes in treatment and provide recommendations.
• Facilitate care coordination to ensure timely access to services.
• Document member information to ensure regulatory compliance.
• Educate members and families on healthcare options and benefits.
• Provide leadership with feedback to improve care delivery.

What You'll Do

• Evaluate member needs via phone or in-home visits and recommend/facilitate care plans
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs
• Develop ongoing care plans for members with high acuity and identify necessary providers and resources
• Coordinate between members, families, community resources, and care provider teams
• Monitor care plans and member status for changes in treatment and provide recommendations
• Facilitate care coordination and collaborate with providers to ensure timely access to services
• Collect, document, and maintain member information to ensure compliance with regulations
• Provide education to members and families on healthcare options and benefits
• Provide feedback to leadership on opportunities to improve care delivery
• Perform other duties as assigned

Requirements

Education

• Master's degree in Behavioral Health or Social Work, or Degree from an Accredited School of Nursing

Experience

• 2 – 4 years of related experience

Licenses & Certifications

• One of the following licenses required:
  – Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC), OR
  – RN based on state contract requirements with BH experience
