Centene is seeking a Care Manager (Behavioral Health) to develop, assess, and facilitate complex care management for members with mental and behavioral health needs, aiming to deliver high-quality, cost-effective outcomes.

Competenze

• Master's degree in Behavioral Health, Social Work or Nursing.
• CA license in BH field or RN license is required by state contract.
• 2–4 years of related experience in care management or BH required.

About This Role

The Care Manager (Behavioral Health) develops, assesses, and facilitates complex care management activities for members with mental and behavioral health needs, aiming to provide high-quality, cost-effective healthcare outcomes.

What You'll Do

• Evaluate member needs via phone or in-home visits and recommend care/service plans.
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs.
• Develop ongoing care plans for members with high acuity and identify necessary resources.
• Coordinate between members, families, community resources, and care provider teams.
• Monitor care plans and member status for changes in treatment and provide recommendations.
• Facilitate care coordination and collaborate with providers to ensure timely access to services.
• Collect, document, and maintain member information for compliance with regulations.
• Provide education to members and families on healthcare procedures and benefits.
• Provide feedback to leadership on opportunities to improve care quality.
• Perform other duties as assigned and comply with all policies and standards.

Requirements

Education

• Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing required.

Experience

• 2 – 4 years of related experience required.

Licenses & Certifications

• One of the following licenses required: LCSW, LMSW, LMFT, LMHC, LPC, or RN based on state contract requirements with BH experience.
• Must be licensed in California.
