Centene is seeking a Care Manager to develop, assess, and coordinate complex care management for members, especially with mental and behavioral health needs, to deliver high-quality, cost-effective outcomes.

This full-time role offers a pay range of $56,200 to $101,000 per year with flexible work options including remote, hybrid, field, or office work, and benefits such as health insurance, 401K, and tuition reimbursement.

Qualifikationen

• Master's in Behavioral Health or Social Work, or RN with BH experience as per contract requirements.
• 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional or RN based on state contract requirements.

Aufgaben

• Evaluate the needs of the member via phone or in-home visits related to available resources.
• Recommend and/or facilitate the care plan/service plan for optimal outcomes.
• Develop ongoing care plans for members with high acuity, identifying necessary providers and community resources.
• Coordinate between the member, family/caregivers, community resources, and care provider team.
• Monitor care plans/service plans and member status for changes in treatment and provide recommendations.
• Facilitate care coordination and collaborate with providers or specialists for timely access to care.
• Collect, document, and maintain member information and care management activities for compliance.
• Provide education to members and their families on healthcare procedures and options.
• Provide feedback to leadership on opportunities to improve care and quality delivery.
• Perform other duties as assigned and comply with all policies and standards.

Requirements

Education:

• Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing

Experience:

• 2 – 4 years of related experience

Licenses & Certifications:

• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience required
