Job summary

Centene is seeking a Care Manager to develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs. The role emphasizes coordinated care plans, education, and collaboration with families and providers.

The position requires a Master's in Behavioral Health or Social Work (or related nursing degree) and 2–4 years of experience, with licensure options including LCSW/LMSW/LMFT/LMHC/LPC or RN as per state contract.

Qualifications

• Master's degree in Behavioral Health or Social Work, or a Degree from an Accredited School of Nursing.
• 2–4 years of related experience.
• Licenses & Certifications: Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC), OR RN based on state contract requirements with BH experience.

Responsibilities

• Evaluate member needs via phone or in-home visits and recommend/facilitate care plans.
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs.
• Develop ongoing care plans for members with high acuity and identify necessary providers and resources.
• Coordinate between the member, family/caregivers, community resources, and care provider team.
• Monitor care plans and member status for changes in treatment and provide recommendations.
• Facilitate care coordination and collaborate with providers or specialists for timely access to care.
• Collect, document, and maintain member information and care management activities.
• Provide education to members and families on procedures, treatment options, and healthcare benefits.
• Provide feedback to leadership on opportunities to improve care and quality delivery.

• Schedule: Full-time, Day shift
