Job summary

Qualifications

• Bachelor’s degree in psychology, social work, or a related field.
• At least 1 year of Case Management experience for Care Manager II; 4 years as Care Coordinator I in related fields.
• Valid driver’s license; CPI/CPR certification may be required.

Responsibilities

• Provide case coordination, outreach, and advocacy to individuals entering CCBHC.
• Perform initial screening and referrals to connect consumers with resources.
• Develop and revision of psychosocial evaluations and treatment plans; coordinate with care team.
• Coordinate with community systems to facilitate linkage, crisis management, and follow-up.
