Community Care of North Carolina (CCNC) seeks a Care Manager 1 - Non-Clinical to provide statewide care management for Medicaid enrolled members in North Carolina. You will assess needs, plan and coordinate services to ensure integrated care and better outcomes.

Residency in NC (within 40 miles of the NC border) and collaboration with primary care providers, families, and a multidisciplinary team are essential. Some travel and remote work from home when not in the field are required.

Formación

• Requires a Bachelor's Degree in a field related to health, psychology, sociology, social work, nursing, or licensure as an RN.

Responsabilidades

• Provide integrated whole-person care management across physical and behavioral health and other services.
• Assess, plan, implement, coordinate, monitor, and evaluate care to Medicaid enrolled members.
• Collaborate with members, guardians, family, providers, and care teams to coordinate a full continuum of health care services.
