Job summary

CPC Integrated Health is seeking a full-time Vocational Specialist to join the PACT Team in Red Bank, NJ. The role focuses on developing and executing holistic care plans in collaboration with consumers within a multi-disciplinary clinical setting.

Responsibilities include assessments, relapse prevention, job coaching, resume writing, and advocacy for employment opportunities. Travel and community transportation training are part of the job.

Qualifications

• Bachelor's degree in behavioral science or related area with 2 years of vocational assessment/placement/job coaching experience for SMI clients.
• Valid New Jersey driver's license with clean record.
• Master's degree in Psych Rehab or Occupational Therapy preferred.

Responsibilities

• Complete assessments or assist staff in completing assessments in a timely, comprehensive manner per CPC policies.
• Collaborate with consumers and families to provide relapse prevention, daily living skills, employment training, psychoeducation, and consumer advocacy.
• Advocate for employment opportunities with local employers and assist with resume writing.
• Document uniform clinical records accurately and timely per policy.
• Participate in interdisciplinary team process to ensure linkage to requisite services.
• Attend required annual training per DMHS standards.
• Work toward fidelity scale goals for the program.
• Provide on/off-site job coaching.
• Train consumers to use public transportation to and from job sites.

CPC is looking for a full-time vocational specialist to work as part of a multi-disciplinary clinical team that will work in collaboration with the consumer in developing and executing a care plan with a holistic approach in our PACT Team in Red Bank, NJ.

Requirements

• Bachelor's degree in behavioral science related area and 2 years of experience with vocational assessment job preparation or placement/job coaching with SMI clients.

• Master's degree in Psych Rehab or Occupational Therapy is preferred.

MAJOR RESPONSIBILITIES/PERFORMANCE OBJECTIVES:

The Vocational Specialist's responsibilities include but are not limited to:

• Complete assessments and/or assist other staff in completing assessments in a timely and comprehensive manner as required by CPC and regulatory agency policies. Assessments are age appropriate and culturally sensitive and address the needs as outlined in the agency assessment tools.
• Collaborate with consumers and their families to provide the following services; relapse prevention, basic daily living skills including money management; development and support of social and recreational activities; employment training and job acquisition; psycho education and support, family and significant others education and support; consumer advocacy; crisis intervention; housing needs, financial resources; transportation for consumers in agency vehicles with adherence to agency policies and procedures; among others. Work with consumers on goals set forth in individual recovery plans.
• Advocate for PACT consumers in developing employment opportunities with local employers. Teach and assist consumers to write resumes.
• Documentation responsibilities include accurate and timely completion of uniform clinical record documentation as per the agency's policies and procedures.
• Participate in an interdisciplinary team process to ensure the provision of services and/or linkage with requisite services to consumer and family or significant others.
• Attend required annual training as mandated by DMHS standards.
• Understand and work to achieve fidelity scale goals for program.
• On/off site job coaching.
• Travel and train consumers to use public transportation to/from job site.
