Job summary

CPC Integrated Health in New Jersey is seeking an experienced Registered Nurse to support individuals in outpatient behavioral health services. The RN coordinates care, provides health education, and helps clients overcome barriers to wellness, working days with the treatment team.

You will take vitals, document in the EHR, assist with screenings, medical case management, injections, prior authorizations, and emergency response while promoting recovery‑oriented care.

Qualifications

• Current New Jersey RN license required.
• Minimum one year of RN experience.

Responsibilities

• Coordinate care with clients, prescribers, and team members to support goals.
• Take vital signs and record height, weight, BMI in the EHR.
• Complete or assist with health, pain, and nutritional screenings; review results with supervisor and prescriber.
• Provide medical case management, including scheduling and follow-up for medical appointments and referrals.
• Administer and document long-acting injectable medications; maintain medication logs.
• Assist with medication prior authorizations and related administrative tasks.
• Support the Med Line process and ensure accurate documentation.
• Respond to emergencies within the program and provide scope‑of‑practice support.
• Promote wellness, healthy lifestyle choices, and self-management of medical and behavioral health conditions.

CPC Integrated Health is a community-based organization dedicated to providing compassionate, evidence-based care to individuals and families affected by mental health, substance use, and co-occurring disorders. We believe in treating the whole person through integrated recovery-oriented care.

Position Summary

The RN supports individuals receiving outpatient behavioral health services by coordinating care, providing health education, and assisting clients in reducing barriers to achieving their wellness goals. This is a daytime position that primarily provides case management and care coordination. The RN will collaborate with the prescriber, treatment team, and external providers to promote both physical and mental health recovery.

• Current New Jersey RN license required.
• Minimum one year of experience working as an RN.

Maximum Qualifications

• Experience working with individuals with mental health, co-occurring, or substance use disorders preferred.
• Strong communication, organization, and documentation skills.
• Ability to work collaboratively as part of an interdisciplinary team.
• Commitment to promoting integrated and recovery-oriented care.

The position requires prolonged sitting, some bending and stretching. Good eye-hand coordination and manual dexterity sufficient to operate standard office equipment is required. Employees must have normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position. The employee must occasionally lift and/or move objects up to 30 pounds.
