CPC Behavioral Healthcare is seeking a Care Manager to work with adults with mental health and co-occurring disorders in clinics, community settings, and homes. The role focuses on coordinating services, establishing goals, and supporting wellness and recovery.

The Outpatient Care Manager delivers case management, crisis intervention, psychoeducation, and skill-building, collaborating with clients, families, and providers to promote long-term wellness and independence.

Qualifications

  • Bachelor's degree in social work, psychology, human services, or related field.
  • At least 1 year of experience in case management, behavioral health, or social services.
  • Master's degree and CADC are preferred.

Responsibilities

  • Manage a caseload, meet productivity standards and conduct home visits.
  • Collaborate with clients, families, and providers to coordinate transportation and appointments.
  • Deliver psychoeducation to clients and families on mental health, substance use, and coping strategies.
  • Coach clients in daily living, respond to crises, and support housing-related needs and safety plans.
  • Connect clients with peer support and community resources to increase engagement.
  • Maintain accurate records in the EHR and monitor progress toward service plan goals.

Job description

The Care Manager is an integral member of the interdisciplinary team involved with each individual's care at CPC. The Care Manager works directly with adult clients experiencing mental health and co-occurring disorders in clinic-based environments, community settings, and clients' homes to coordinate services, establish goals, and support individuals in achieving wellness, recovery, and productive lives. This position involves field-based outreach to ensure access to essential services and reduce barriers to care.

The Outpatient Care Manager actively delivers case management, crisis intervention, psychoeducation, and skill-building support, directly connecting individuals to vital community resources. They work collaboratively with clients, their families, support systems, clinical teams, and community providers to achieve positive outcomes and promote long-term recovery.

Title: Care Manager

MINIMAL QUALIFICATIONS:

  • Education: Bachelor's degree in social work, psychology, human services, or a related field required.
  • Experience: Minimum of one (1) year of experience in case management, behavioral health, or social services.

PREFERRED QUALIFICATIONS:

  • Two or more years of direct experience working with people experiencing Serious and Persistent Mental Illness (SMI) or significant co-occurring mental health and substance use in a home-based or community outreach setting preferred.
  • Master's degree preferred.
  • CADC preferred.

MAJOR RESPONSIBILITIES:

  • Manage a caseload, meet productivity standards. Conduct home visits and community-based outreach to engage with clients, assess evolving needs, and support continuity of care.
  • Collaborate with clients, Guide and support clients, Coordinate transportation and, when appropriate, accompany clients to key appointments to foster empowerment and follow-through.
  • Deliver psychoeducation to clients and families on mental health, substance use, and healthy coping strategies, with a focus on addressing co-occurring mental health, substance use, or physical health challenges.
  • Coach clients in building daily living, respond to crises within the clinic setting, partner with clients on housing-related needs and collaborate on safety plans, relapse prevention tools, and wellness strategies that promote long-term recovery.
  • Connect clients with peer support, community programs, and natural support systems to increase engagement and independence.
  • Maintain clear, timely, and accurate records in the electronic health record (EHR) system, in accordance with regulatory standards.
  • Monitor and document client progress toward service plan goals, contributing to ongoing program improvement.
  • Uphold compliance with all federal, state, and local regulations, including those related to Medicaid and grant-funded services.
  • Ensure strict adherence to HIPAA regulations and organizational privacy policies to protect client confidentiality and health information.

ESSENTIAL COMPETENCIES:

  • Ability to work in a clinic setting, homes, and other non-clinical environments.
  • Strong understanding of mental health, substance use, and social determinants of health.
  • Excellent problem-solving and organizational skills.
  • Ability to engage and build trusting relationships with diverse populations.
  • Proficiency in electronic health records (EHR), Microsoft Office, and online resource navigation.
  • Commitment to trauma-informed, person-centered, and culturally competent care.
  • Flexibility in scheduling to meet client and program needs.
  • An ability to independently travel to a variety of locations using a personal or agency-provided vehicle.
