Job summary

Shriners Children's in the United States seeks a Care Manager to coordinate and facilitate comprehensive, patient-centered care for children with complex medical needs, ensuring optimal outcomes and a seamless experience for families.

You will perform holistic assessments, identify non-clinical supports like housing and travel, develop individualized care plans with the interdisciplinary team, and support resource management to optimize patient outcomes.

Qualifications

• RN license in state of practice.
• BLS certification within 6 months of hire.
• Preferred experience in acute care, pediatrics, or specialty case management.
• Knowledge of care coordination models and regulatory guidelines (CMS, URAC), CCM/ACM is a plus.

Responsibilities

• Perform comprehensive assessments to identify patient and family needs early in care.
• Identify travel, housing, and other non-clinical resources to facilitate access to care.
• Develop individualized care plans with the interdisciplinary team to optimize outcomes.
• Balance resources with patient preferences to support informed decisions.
• Support education and training for the interdisciplinary team.

Job Summary

The Care Manager at Shriners Hospitals for Children is responsible for coordinating and facilitating comprehensive, patient-centered care for children with complex medical needs, ensuring optimal outcomes and a seamless healthcare experience for patients and families.

Key Responsibilities

• Perform comprehensive assessments to identify patient and family needs, early in the care process, to ensure smooth progression throughout the continuum of care.
• Identify logistical needs, including travel, housing, and other non-clinical resources to facilitate access to care and refer to appropriate support staff.
• Develop individualized, goal driven care plans in collaboration with the interdisciplinary team to optimize patient outcomes.
• Balance available resources with patient preferences, to facilitate informed decision making.
• Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to support appropriate resource management.
• Support interdisciplinary team education and training.

Required Qualifications

• RN license in state of practice
• Basic Life Support (BLS) Required within 6 months of hire
• Preferred Qualfications 1-2 Acute Care/Pediatrics/Specialty Case Management
• Preferred Knowledge of care coordination models and regulatory guidelines (e.g., CMS, URAC) CCM, ACM BSN
