Job summary

Piedmont Newnan Hospital is seeking a Care Manager to oversee care coordination, progression of care, and proactive discharge planning for patients in their caseload. The role requires RN licensure in Georgia, with a Master’s degree preferred and at least two years in acute or post-acute settings.

Familiarity with interQual or Milliman is preferred. This position supports a collaborative hospital culture and aims to improve patient flow and outcomes through structured discharge planning and care

Qualifications:

Education

• Graduate of Nursing Program Required
• Master's degree Preferred

Work Experience

• 2 years of experience in an acute or post-acute setting Required
• Experience using InterQual or Milliman Preferred

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
• Certification in Case or Care Management Preferred
