Job summary

Piedmont Newnan Hospital is seeking a Care Manager to lead care coordination, progression of care, and proactive discharge planning for a defined caseload. You will collaborate with nurses, physicians, social workers and ancillary staff to ensure safe, timely transitions.

The role requires a registered nurse with at least two years in acute or post-acute settings, strong knowledge of care management principles, and the ability to advocate for patients while navigating InterQual or Milliman

Qualifications

• Graduate of Nursing Program required.
• Georgia RN licensure required or multi-state license.
• 2+ years of experience in an acute or post-acute setting required.
• Experience with InterQual or Milliman preferred.
• Care management certification preferred.

Responsibilities

• Care Manager coordinates care and progression of care including proactive discharge planning.
• Expedite timely and safe discharge for all patients in their caseload.

Qualifications:

Education

• Graduate of Nursing Program Required
• Master's degree Preferred

Work Experience

• 2 years of experience in an acute or post-acute setting Required
• Experience using InterQual or Milliman Preferred

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
• Certification in Case or Care Management Preferred
