Job summary

Piedmont Newnan Hospital is seeking a dedicated Care Manager to coordinate patient care, progression of care, and proactive discharge planning. You will be accountable for expediting timely and safe discharges for all patients in your caseload, collaborating with interprofessional teams to optimize outcomes.

The ideal candidate is an RN with Georgia licensure, at least 2 years in acute or post-acute settings, and certified or pursuing certification in Case/Care Management.

Qualifications

• RN with GA state licensure required.
• Graduate of nursing program required; master's preferred.
• Minimum 2 years in acute or post-acute setting required.
• Experience with InterQual or Milliman preferred.
• Certification in Case or Care Management preferred.

Responsibilities

• Coordinate care and progression of care across settings.
• Proactively plan discharges and expedite timely, safe discharges.

Qualifications

Education

• Graduate of Nursing Program Required
• Master’s degree Preferred

Work Experience

• 2 years of experience in an acute or post-acute setting Required
• Experience using InterQual or Milliman Preferred

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
