Job summary

Piedmont Rockdale Hospital in Georgia seeks an experienced Care Manager RN to coordinate care, progress patient treatment, and plan efficient discharges. You will lead care coordination, ensure timely progression of care, and partner with interdisciplinary teams to safeguard patient safety and quality outcomes.

The role requires an active Georgia RN license and at least 2 years in acute or post-acute settings; a Master’s degree is preferred, with familiarity in InterQual or Milliman tools

Qualifications

• Graduate of Nursing Program required.
• Master's degree in Nursing preferred.
• 2 years of experience in an acute or post-acute setting required.
• RN license for Georgia required.
• Certification in Case or Care Management preferred.

Responsibilities

The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load.

Qualifications

Education

• Graduate of Nursing Program Required
• Master's degree Preferred

Work Experience

• 2 years of experience in an acute or post-acute setting Required
• Experience using InterQual or Milliman Preferred

Licenses and Certifications

• RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
• Certification in Case or Care Management Preferred
