Job summary

MonmouthCares Inc. is seeking a Care Manager to facilitate Wraparound services for children with emotional and behavioral challenges and their families in partnership with NJ System of Care providers.

You will coordinate assessments, develop ISP plans, and ensure regular progress updates and family engagement. Responsibilities include facilitating Child Family Teams, coordinating with school, probation, and mental health services, and connecting families to housing, services, and supports.

Qualifications

  • - Education: BA/BS degree in a relevant field (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, special education) with one-year related experience or a Master’s Degree in a related field.
  • - MS degree desirable
  • - LSW licensure desirable
  • - Bilingual Spanish, Portuguese and/or Creole is a plus

Responsibilities

  • Coordinate and facilitate the Wraparound process with children and families.
  • Identify family strengths and needs and complete Strength and Needs Assessments.
  • Develop Individual Service Plans (ISP) based on identified needs and strengths.
  • Facilitate Child Family Teams to create strength-based strategies in ISPs.
  • Maintain regular contact with system partners to monitor ISP progress and update plans.
  • Update ISP at least every 90 days based on assessments.
  • Engage families and providers and connect families to community resources.
  • Advocate for families by attending meetings (school, court hearings, mental health appointments).
  • Provide on-call coverage to ensure 24/7 agency availability.
  • Complete Medicaid-required documentation and meet agency standards.

Description

Position Summary

To facilitate positive change in children with emotional and behavioral challenges along with their families, following the Wraparound process and the strength-based philosophy in partnership with stakeholders and service providers in the NJ System of Care network.

Key Responsibilities

The Care Manager coordinates and facilitates the Wraparound process of working with children and families utilizing a strength-based team approach to ensure family’s needs are addressed and met.

The responsibilities include:
  • Applying wraparound model and process in carrying out the care management duties.
  • Helping families identify their strengths and needs and completing the Strength and Needs Assessments.
  • Using the strengths and needs of the child and family to guide development of Individual Service Planning (ISP)
  • Facilitating the Child Family Teams in developing the Individual Service Plans that identify strength-based strategies to address the child and family’s needs.
  • Collaborating and maintaining regular contact with various system partners in order to monitor progress of and update the ISP. (e.g., DCP&P, Probation, Juvenile Justice, school systems).
  • Updating the Individual Service Plan (ISP) at least every 90 days based on ongoing assessment to determine if the interventions are implemented effectively and are generating positive outcomes.
  • Ongoing engagement with the family and providers
  • Connecting families to available resources in the community, including housing, rental assistance, social service entitlements, domestic violence, outpatient mental health and substance abuse treatment facilities, food pantries, etc.
  • Advocating on behalf of families by attending meetings with families (school, DCP&P, court hearings, mental health appointments) to ensure the family’s voice is heard.
  • Serving on-call duty on a rotating basis to ensure 24/7 coverage by the agency.
  • Completing the following activities and deliverables per Medicaid requirements and Agency standards:
  • Weekly communication with families
  • Family visits at least once a month or more as needed
  • Bi-monthly communication with CFT members
  • Completion of 7-day Family Crisis Plans, 30-day Initial CFT, 90-day Comprehensive CFT
  • Documentation of progress notes within 72 hours of the activity
  • Knowledge Monmouth Cares Mission Wraparound philosophy, principles, and process
  • New Jersey System of Care – processes and partners
  • MIS - standard reporting, documentation, referral, and authorization processes
  • Broad understanding of our customer base (i.e. children, families, human service agencies, schools, etc.).
  • Knowledge of community resources
  • Child development and psychology
  • Child/adolescent behavioral health and management (including DSM-IV)
  • Knowledge of educational system, including special education
  • Knowledge of child welfare system
  • Knowledge of Juvenile justice system
  • Knowledge of child protection services
