Job summary

Trusted Path in Maryland is seeking an experienced RN for a part-time role focused on in-home geriatric care. You will perform nursing and geriatric assessments, develop care plans, and coordinate with physicians, therapists, and families.

There are no fixed shifts and nurses can work from home for coordination and documentation, scheduling client visits to fit personal lives. Flexible scheduling, meaningful one-on-one client relationships, and autonomy to arrange visits around your calendar are

Qualifications

  • Active Maryland RN license.
  • 5+ years of nursing experience.
  • Strong assessment, communication, and organizational skills.
  • Ability to work independently and manage your own schedule.
  • Reliable transportation for local client visits.
  • Geriatric, case management, home health, neurology, psychiatry, hospice, or community nursing experience preferred.

Responsibilities

  • Complete nursing and geriatric assessments.
  • Develop and update individualized care plans.
  • Coordinate care with physicians, therapists, home health agencies, caregivers, and other providers.
  • Accompany clients to medical appointments when needed.
  • Provide medication management, health monitoring, and client/family education.
  • Support clients with dementia, Parkinson’s disease, and other age-related conditions.
  • Identify home safety and fall-prevention needs.
  • Help families navigate healthcare services and community resources.
  • Advocate for clients and communicate with families and healthcare providers.
  • Complete timely documentation and ongoing care coordination.

Role Description
Why This Role Is Different

This position offers significant flexibility and autonomy. There are no traditional set shifts or office hours. Nurses can work from home for care coordination and documentation and schedule client visits on days and times that work with their personal lives while meeting client needs.
