Job summary

New Mexico Solutions in Albuquerque is seeking a Care Management Supervisor to lead the Care Management team, oversee care coordination, and support program operations.

The role includes direct client care, staff supervision, development of care plans, and collaboration with multidisciplinary teams to ensure compliant, person-centered service delivery. A high school diploma with relevant experience or higher education is required.

Qualifications

• Master’s Degree in a human services related field OR
• Bachelor’s degree in a human services related field and 1 year relevant experience OR
• High School Diploma or GED with a minimum of 3 years of experience working with the target population OR
• Certified Peer Specialist (CPS) or Certified Family Specialist (CFS)

Responsibilities

• Performs all essential functions of the Community Care Manager, including care management, care coordination, direct service delivery, and group facilitation.
• Provides individual interventions to help clients achieve person-centered goals.
• Coordinates services across behavioral health, physical health, and community providers.
• Leads multidisciplinary treatment planning and safety/relapse prevention efforts.
• Engages clients in crisis response and follow-up services.
• Leads and mentors Care Management staff; supports program growth and compliance.
