Job summary

New Mexico Solutions, based in Albuquerque, NM, seeks a Care Manager – Crisis Services to provide person-centered care coordination for individuals in behavioral health crises. You will assess risks, plan services, coordinate across systems, and advocate for client needs within a multidisciplinary team.

In this role, you will work closely with clinical staff, medical providers, and community partners to support stabilization, access to appropriate care levels, and recovery-oriented outcomes.

Qualifications

• Master’s Degree in a human services field or higher.
• Bachelor’s degree with 1 year relevant experience preferred or HS diploma with 3+ years experience.

Responsibilities

• Engage individuals in acute behavioral health crises using trauma-informed approaches.
• Coordinate care plans and referrals across systems of care.
• Maintain timely documentation in electronic health records.
• Collaborate with clinical staff and community partners to support stabilization and recovery.
