$23.00/hr - $24.00/hr

Immediate need for a talented Care Management Support Coordinator II. This is a 06+ months contract opportunity with long-term potential, located in US (Remote PST). Please review the job description below and contact me ASAP if interested.

Pay Range: $23 - $24/hour. Employee benefits include health insurance (medical, dental, vision), 401(k), and paid sick leave (depending on work location).

Key Responsibilities:
  • Shift: 7 am – 4 pm PST / 8 am – 5 pm PST. Availability for weekend work as needed. OT not required.
  • Schedule breaks and lunches in a live call center environment.
  • Support administrative care management activities including outreach, inbound calls, and scheduling.
  • Serve as a point of contact for members, providers, and staff to resolve issues and document member records per regulations.
  • Assist members in connecting to health plan and community resources to ensure quality care.
  • Apply knowledge of assigned health plan activities and resources.
  • Handle member and provider inquiries, explain care procedures, and protocols.
  • Support member onboarding and administrative duties like sending welcome letters and educational materials.
  • Maintain non-clinical member records adhering to policies and regulations.
  • Knowledge of local benefits and resources, addressing Social Determinants of Health (SDOH).
  • Perform other duties as assigned and comply with policies.
Key Requirements and Technology Experience:
  • Skills: Call Center, Scheduling, Healthcare
  • Software: Microsoft Office (Outlook)
  • 1-2 years related experience supporting outreach, scheduling, and education for members and providers.
  • Ability to work from home in a fast-paced environment, collaborating with clinical teams.
  • Experience in scheduling and peer-to-peer communication.
  • Critical thinking, adaptability, and receptiveness to feedback.
  • Call metrics: avg handle time 7.5 min, hold time 1.5 min, 90% service levels, 95-100% audit scores, 20-40 calls/day.
  • High school diploma or equivalent.
