Job summary

AdventHealth Palm Coast seeks a qualified Case Manager to support discharge planning and care coordination across the patient care continuum. Knowledge of medical necessity, CMS Conditions of Participation, and interdisciplinary teamwork will guide daily decisions.

Requirements include a Bachelor's degree in Social Work (Master's preferred), 1+ year direct care management experience, and strong communication skills. This is a full-time day shift with benefits from day one.

Qualifications

• Knowledge of medical necessity for appropriate patient status and level of care (Required).
• Knowledge of IMM, MOON, Advanced Directives, Multidisciplinary Rounds, Social Services, Value Based Programs, and other Care Management compliance guidelines (Required).
• Ability to lead others effectively while maintaining appropriate compliance (Required).
• Knowledge of CMS Conditions of Participation for discharge planning (Required).
• Ability to manage multiple tasks and prioritize level of importance (Required).
• Critical thinking skills (Required).
• Customer service skills (Required).
• Ability to implement process improvement processes (Required).
• Experience with PC applications and development of reports (Required).
• Ability to negotiate optimal resolution of issues (Required).
