Job summary

Carolina Outreach is seeking a Supervising Care Manager to lead a team of 8 care managers and oversee care management services for behavioral health members in Supply, NC. The role is salaried and focuses on performance coaching, outcomes, and adherence to program standards.

The Supervising Care Manager will supervise daily operations, review assessments and care plans, monitor compliance with 1915(i) services, and support member engagement to meet contract requirements.

Qualifications

• Master's-level degree in a field related to health, psychology, sociology, social work, or another relevant human service.
• Licensure as an RN.
• Three years of experience providing care management, case management, or care coordination to the population served.

Responsibilities

• Oversee daily operations and performance of the Care Management team.
• Provide supervision, coaching, and guidance to Care Managers.
• Review assessments and care plans to ensure quality and accuracy.
• Monitor compliance, member engagement, and progress toward care goals.
