Qualifications

• Bachelor’s degree in a human service, social science, or health field.
• Five years’ care management experience, with elderly/disabled exposure.
• Knowledge of long-term care services and regulatory requirements.
• Proficient in documentation and case management software.
• Proficient in Microsoft Office applications.

Responsibilities

• Lead and supervise a care management team to deliver high-quality, person-centered care.
• Monitor activity, productivity, and caseload performance to ensure timely work products.
• Ensure compliance with policies, contracts, and state/federal regulations through audits.
• Provide coaching, training, and professional development to staff.
• Review dashboards and data to identify trends and drive process improvements.
• Collaborate with providers, families, and interdisciplinary teams to support care plans.
• Promote member satisfaction and continuity of care through responsive service.
