Responsibilities

• Assesses discharge planning needs in inpatient, observation and ED settings to avoid delays.
• Develops discharge plans with contingency plans for post-acute care and social needs.
• Coordinates with post-acute services to ensure complete medical records and accurate discharge reconciliation.
• Participates in multi-disciplinary rounds to review patient status and escalation when needed.
