Responsibilities

• Collaborates on design, implementation, and evaluation of care and disease management projects.
• Acts as primary contact and resource for specified projects; manages contracts and stakeholder communication.
• Prepares and presents reports on project performance for internal and external meetings.
• Develops project plans and manages timelines using existing tools.
• Coordinates with CHNCT departments and ASOs to ensure access to care/services.
• Supports URAC audit activities and accreditation processes.
