Job summary

Baptist Health Princeton Hospital is seeking a Care Management Supervisor to lead the care management team, oversee discharge planning, and coordinate services for safe patient transitions. This role drives resource utilization and improves patient outcomes through strong leadership and collaboration within Baptist Health.

Ideal candidates will have nursing or MSW/MHC/MFT background, 3 years in care management, and a track record in utilization management and regulatory compliance.

Qualifications

• Graduate of an approved nursing program required.
• MSW, MHC or MFT at master’s level required depending on role.
• 3 years in chronic disease management, care management, care coordination, utilization management, or acute clinical care required.
• 2 years in utilization management preferred.
• RN license (Alabama) required if nursing trained.

Responsibilities

• Lead care management teams to ensure high-quality, cost-effective, patient-focused services.
• Coordinate workload and staffing within the Baptist Health network.
• Develop relationships with site leadership, medical staff, and payors.
• Support quality and cost-improvement initiatives to prevent readmissions.
• Ensure regulatory compliance with NCQA, HIPAA, CMS and others.
