Qualifications

• Licensed RN; MSW/MHC/MFT advanced degrees where applicable.
• Experience in chronic disease management, care coordination and utilization management.
• Ability to lead teams, manage budgets and improve care processes.

Responsibilities

• Directs care management teams to deliver high quality, cost effective, patient‑focused services.
• Oversees staffing, scheduling and resource allocation within Baptist Health network.
• Develops relationships with site leadership, medical staff, vendors and community partners.
• Drives performance improvement and population health initiatives across care transitions.
• Ensures compliance with regulatory standards (NCQA, HIPAA, CMS and others).
• Manages budget related to payroll, supplies and department operations.
