Job summary

Piedmont Healthcare is seeking a Care Manager to lead the Care Management team at Piedmont Atlanta Hospital, coordinating patient care across the continuum.

You will mentor and coach Care Management professionals, prioritize complex workloads, drive discharge planning, and partner with physicians, nursing leaders, payers, and community organizations to improve outcomes and transitions of care.

Qualifications

• Graduate of Nursing Program required.
• Master's degree preferred.
• 2 years of experience in an acute or post-acute setting required.
• Current unrestricted RN license in Georgia required.
• Certification in Case or Care Management preferred.

Responsibilities

• Care coordination across the continuum and progression of care.
• Proactive discharge planning and timely safe discharge for patients in caseload.
• Lead, coach, and mentor Care Management professionals.
• Build partnerships with physicians, nursing leaders, payers, and community organizations.
