Job summary

Cherokee Indian Hospital Authority is seeking a Tribal Option Care Management Extender to support care coordination for members eligible for Tailored Care Management, I/DD/TBI, LTSS, CFSP, and related programs. The role emphasizes person-centered planning, referrals, and follow-up within EBCI policies.

The candidate will collect data, document in EHR, and collaborate with care teams to link clients to essential services, with a focus on timely follow-up and safety while working in a hybrid

Qualifications

• Care extenders provide care management functions per policy.
• Gather and document information in EHR and care management platform.
• Engage members in person-centered planning and link to needed services.

Responsibilities

• Assist with person centered planning, referral and linkage, and monitoring and follow-up.
• Ensure care plan activities are carried out under supervision.
• Coordinate discharge planning and follow-up care with care team.
• Document significant events in EHR and care management platform.
