The role involves conducting interviews with members, processing referrals, and providing non-clinical support to Care Managers. Ideal candidates will have strong customer service skills, be bilingual in Spanish, and possess a HS Diploma or GED.
Job Description
The Care Management Coordinator will assist assigned team, nurse or social worker with the coordination of health care activities for members. Conducts telephone interviews with member, significant others and family members to determine if care needs are being met or if additional services are needed. Direct members to available resources and educate patients on a wide range of programs and options available to them. Review and process all referrals as well as communicate with providers, members, PCP or referring parties for services. Creates and processes program referrals. Schedules assessments coordinates and obtains medical records, ensuring processes are completed within regulatory timelines. Documents all interventions, follow up information and communication with members in the members EMR. Provides non-clinical support to Care Managers. Establishes telephonic, electronic correspondence andor fax contact with internal and external customers following internal policies and HIPAA guidelines. Other duties as assigned.
Skills Required
HS Diploma or GED. Prior administrative, customer service or data entry exp. Bilingual in Spanish. Proficient in MS Office. Able to type 45 WPM
Skills Preferred
Previous exp in Care Management or healthcare.
