Job summary

Jobtailor is seeking an experienced care coordination professional to support youth and families within OhioRISE programs. You will lead care teams, facilitate collaboration across families, communities, and service providers, and ensure compliant documentation and service delivery in various settings.

Responsibilities

  • Create, plan, and organize a team as defined by the family.
  • Develop, lead, and facilitate team meetings for the young person and their identified team.
  • Provide care coordination within the family’s home, community, or preferred location.
  • Partner with young people, families, natural supports, and other service providers to identify care coordination needs and services.
  • Ensure the best outcomes for young people and families.
  • Ensure youth and families have voice and choice in coordinated care and services.
  • Maintain required documentation to access and maintain OhioRISE benefits.
  • Meet quality and fidelity requirements defined by ISBH OhioRISE standards.
  • Acquire and maintain knowledge of community resources.
  • Attend mandatory trainings and supervisions, requiring travel.
  • Complete assigned trainings within identified timelines and maintain required certifications.
  • Follow policies regarding confidentiality, professionalism, and incident reporting.
  • Utilize databases and Electronic Health Record systems for accurate client documentation.
  • Maintain compliance with HIPAA and mandated reporting requirements.
  • Travel within assigned service area.
  • Perform other duties as assigned

Job description

  • Perform other duties as assigned
Requirements
  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field is required: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
  • Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment)
  • Two years of experience in a coordinated supportive services or care coordination role that required high efficiency, organization, and attention to detail is preferred
  • Experience working with people with autism spectrum disorders and developmental disabilities is preferred
  • Experience in one or more of the following areas is required: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience needed to work with children and families experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and engaged with one or more child serving systems
  • Effective verbal/written communication required
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings required
  • Ability to maintain confidential information required
  • Ability to adhere to all safety, rules, regulations and requirements required
  • Ability to bill Medicaid without restrictions required
  • Valid driver's license and ability to operate a motor vehicle required
  • Appropriate level of auto insurance coverage required
  • Ability to manage deadlines within a fast-paced, high-volume environment required
  • Ability to operate in an Internet-based, automated office environment required
  • Ability to operate, receive and conduct on-camera video calls and/or meetings in an automated office environment required
  • Ability to maintain a minimum of 100mbps high-speed internet connection required
  • Prolonged periods of sitting at a desk and working on a computer required
  • Must be able to lift up to 15 pounds at times
