Job summary

Comagine Health in the United States is seeking a dedicated case manager to support patient access and care coordination within our health system collaborations. You will screen and assess cases, develop personalized care plans, and monitor progress in collaboration with physicians and care teams.

Responsibilities include service planning, facilitating interventions, advocating for patients and families, and ensuring nonclinical staff deliver appropriate support under supervision.

Qualifications

• Possess a baccalaureate or graduate degree in social work, or other health or human services field.

Facilitate patient access to quality, cost-effective health care.

Develop and monitor patient-specific goals and interventions in collaboration with attending physician and health care team.

Screen and assess case management patients.

Develop a specific patient centered care plan, complete with short and long-term goals.

Plan and complete interventions, service coordination and implementation, related to the care plan.

Provide patient and family advocacy during the case management process.

As appropriate, provide daily oversight and monitoring of non-clinical staff during their performance of nonclinical support activities; also provide the supervisor with input regarding employees' performance of these activities.

Possess current and active licensure or certification in a health or human services discipline with a legal scope
