Job summary

Comagine Health is seeking a Registered Nurse Case Manager to facilitate access to quality, cost-effective care in Washington state. You will develop patient-centered plans, monitor progress, and coordinate care with the healthcare team.

Preferred candidates hold CCM certification and have strong motivational interviewing skills, along with current licensure and relevant clinical experience. This role is on-site and focused on improving outcomes for diverse patient populations.

Qualifications

• RN license required and active in health or human services field.
• URAC credential standards familiarity or compliance preferred.
• CCM certification preferred.

Responsibilities

• Screen and assess case management patients to determine needs.
• Develop patient-centered care plans with short and long-term goals.
• Coordinate services and interventions to implement the care plan.
• Provide patient and family advocacy throughout the case management process.
• Supervise non-clinical staff as appropriate and provide performance input to supervisor.

Purpose of Position:

• Facilitate patient access to quality, cost-effective health care.
• Develop and monitor patient-specific goals and interventions in collaboration with attending physician and health care team.
• Provide reports on findings, recommendations, and outcomes.

Typical Duties:

• Screen and assess case management patients.
• Develop a specific patient centered care plan, complete with short and long-term goals.
• Plan and complete interventions, service coordination and implementation, related to the care plan.
• Provide patient and family advocacy during the case management process.
• As appropriate, provide daily oversight and monitoring of non-clinical staff during their performance of nonclinical support activities; also provide the supervisor with input regarding employees' performance of these activities.

Preferred experience with motivational interviewing.

Possess current and active licensure or certification in a health or human services discipline with a legal scope of practice that permits independent assessment.
