Job summary

COMAGINE HEALTH in Alaska is seeking a case manager to coordinate care, facilitate patient access to quality services, and develop patient-centered care plans in collaboration with the health care team.

Candidates must possess licensure in a health or human services field or meet state requirements; a BS/BA in social work or related field is preferred. This role emphasizes advocacy and coordinating non-clinical support to improve outcomes.

Responsibilities

Facilitate patient access to quality, cost-effective health care.

Develop and monitor patient-specific goals and interventions in collaboration with attending physician and health care team.

Screen and assess case management patients.

Develop a specific patient centered care plan, complete with short and long-term goals.

Plan and complete interventions, service coordination and implementation, related to the care plan.

Provide patient and family advocacy during the case management process.

As appropriate, provide daily oversight and monitoring of non-clinical staff during their performance of nonclinical support activities; also provide the supervisor with input regarding employees' performance of these activities.

Possess current and active licensure or certification in a health or human services discipline with a legal scope
