Qualifications

• Bachelor's degree in healthcare field or equivalent work experience; total 4 years in direct patient care, social work, quality improvement or health coaching.
• 2+ years of direct patient care, social work, quality improvement or health coaching experience; managed care environment preferred.

Responsibilities

• Close care gaps through outreach and education with members and providers.
• Schedule preventive services and coordinate transportation as needed.
• Educate providers on HEDIS measures and coding per NCQA requirements.
• Collaborate with Provider Relations to improve provider performance in Quality and Risk Adjustment.
• Maintain confidentiality of protected health information (PHI).
