The Care Integration Senior Aide (CISA) conducts in-home visits to work directly with clients and Direct Support Professionals (DSPs). The CISA observes, documents, and reports changes in clients’ health and social conditions, collaborates with the care team to help prevent avoidable negative outcomes, and supports quality service delivery throughout the Community Supported Living (CSL) program.
This position provides guidance and support to DSPs but does not have supervisory responsibilities, including scheduling, performance management, or disciplinary functions.
