Job summary

Qualifications

• High school diploma or equivalent.
• One year of caregiver experience with the agency preferred, or two years of caregiver experience in a similar setting.
• Reliable transportation, a valid driver’s license, and proof of current automobile insurance.
• Ability to speak, read, and write functional English sufficient to perform essential job duties and complete required documentation.

Responsibilities

• Conduct in-home visits to observe and document clients’ health and social needs.
• Communicate observations, concerns, and updates to the interdisciplinary care team.
• Build positive relationships with clients, families, caregivers, and community partners.
• Adhere to agency policies, professional standards, and documentation requirements.
• Provide direct support to CSL members as needed within the scope of the role.
