Qualifications

• Healthcare experience required, preferably in home health or hospice intake/referrals.
• Strong knowledge of insurance verification, authorizations, and reimbursement processes.
• Excellent communication, customer service, and relationship-building skills.
• Nursing background preferred, but not required.

Responsibilities

• Oversee daily intake and referral operations to ensure timely patient admissions.
• Verify insurance eligibility, authorizations, and reimbursement accuracy.
• Coordinate patient transitions to home health services.
• Provide direct oversight of patient education and preparation for home health, plan of care initiation, and coordination of care with multiple service providers.
• Maintain strong relationships with referral sources, case managers, and healthcare partners.
• Track and report referral trends and intake metrics.
• Support pricing discussions and payer communication as needed.
