Job summary

Care Finders Total Care in Jersey City, NJ is seeking a Care Coordinator Supervisor to oversee care coordination, supervise coordinators, manage referrals, and ensure timely service delivery.

Requirements include a High school diploma, 3+ years of care coordination experience, 1+ years using a Home Care Platform such as HHA Exchange, and bilingual proficiency in English and Spanish. Schedule is Mon-Fri 8:30AM-5:00PM with location in Jersey City.

Qualifications

• 3+ years of care coordination experience required.
• 1+ years of experience using a Home Care Platform or scheduling system such as HHA Exchange.
• Bilingual in English & Spanish required

Responsibilities

• Supervise and support Care Coordinators, including assigning tasks, monitoring workloads, and ensuring compliance with policies and procedures.
• Oversee patient intake and care coordination processes, ensuring continuity, quality of care, and timely service delivery.
• Receive and review referrals from healthcare providers, ensuring documentation is complete and accurate.
• Manage caregiver scheduling and staffing coverage based on authorized hours, caregiver skills, and client preferences.
• Serve as the primary point of contact for patients, families, caregivers, and healthcare teams regarding care coordination needs.
• Communicate patient updates, care plans, and scheduling changes while maintaining accurate and confidential records.
• Participate in care conferences, resolve service barriers, and support billing and service verification processes.
• Mentor and provide guidance to care coordination staff on complex cases and day-to-day operations.
• Educate patients and families on available services, community resources, and care expectations.
• Support quality improvement initiatives, data collection, compliance efforts, and participate in on-call rotation as needed.
• Perform other duties as assigned.
