Job summary

Peerstar LLC is seeking an experienced Care Coordinator to join our Operations Team in the Pennsylvania/New Jersey region. The role links service delivery, staffing, and peer engagement, coordinating services, matching peers with Certified Peer Specialists, managing schedules, monitoring authorized hours, and ensuring continuity of care.

Candidates with home health care, Medicaid programs, behavioral health, or healthcare staffing experience are strongly encouraged to apply.

Qualifications

• 1-3 years of experience in Care Coordination, Home Care Scheduling, Healthcare Staffing, Client Services, Case Management Support, or Healthcare Operations.
• Experience in a Home Care/Medicaid program and caregiving scheduling preferred.
• Experience coordinating schedules or staffing field-based employees preferred.
• Strong organizational, communication, and problem-solving skills.
• Ability to prioritize multiple tasks in a fast-paced environment.
• Proficient with Microsoft Office and electronic documentation systems.
• Experience with Medicaid, managed care, authorizations, or healthcare scheduling is a plus.

Responsibilities

• Staff peers with Certified Peer Specialists based on IRP authorization, availability, and geographic needs.
• Monitor utilization to ensure authorized hours are maximized.
• Identify and resolve service gaps or underutilization trends.
• Conduct outreach to support peer engagement and retention.
• Collaborate with CPS and leadership to maintain consistent service delivery.
• Track scheduling trends and recommend operational improvements.
• Maintain compliant documentation and follow HIPAA standards.
