Qualifications

• 3 years of recent clinical experience required, leadership experience preferred. If ADN, 10+ years of experience required.
• Associates Degree OR BSN required upon hire; if Associates, BSN required within 3 years of hire
• Master's degree preferred
• Active PA RN license required

Responsibilities

• Coordinate care across the health care continuum with physicians, peers, community colleagues, and others.
• Develop, implement, and monitor a comprehensive plan of care including health promotion and disease management.
• Lead and be accountable for cross-continuum care coordination and care transitions.
• Engage in hospital committees, unit councils, and staff precepting activities.
