Job summary

Sentara Health is seeking a Care Coordinator to join the Population Health Ambulatory Clinical Support Team in Charlottesville, VA. This hybrid role requires 2-3 days in office and days on-site at affiliate practices, with some work-from-home days.

Responsibilities include case management for patients and families, coordinating multidisciplinary services, and assisting with environmental challenges related to medical conditions.

Qualifications

  • Bachelor's degree required in Health & Human Services or related field.
  • 1 year of Long-Term Care experience required.
  • Licensure as Virginia Pharmacy Tech and CMA/RMA/CCMA preferred; LPN license also considered.

Overview

Sentara is hiring a Care Coordinator for our Population Health Ambulatory Clinical Support Team.

Location: Sentara Quality Care Network Charlottesville

Status: Full Time (40 hours/week)

Shift: Days (Anticipated 8AM-5PM)

*This Hybrid position requires 2-3 days per week in the office, in addition to days on site at affiliate practices, and allows for some work from home days.

The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Education
  • Bachelor's Level Degree (Required)

Must possess a degree in Health & Human Services or one of the following related Fields:

Certification/Licensure
  • ** Ideally seeking a licensed Pharmacy Technician with a Certified Medical Assistant (CCMA) or LPN**
  • Virginia Pharmacy Tech License and CPhT License (Received through the NHA or PTCB)
  • Certified Medical Assistant (CMA), or Registered Medical Assistant (RMA), or Certified Clinical Medical Assistant (CCMA)
  • LPN Virginia state license
Experience
  • 1-year of Long-Term Care experience (Required)
  • Ambulatory and Clinical Pharmacy experience (Preferred)
  • 1-Year Medicaid or Health Plan experience (Preferred)

Compensation

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $24.91 - $40.07 hourly.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
