Traditional Home Care has an immediate opening for a Patient Care Coordinator in Dunmore, PA. This salaried, weekly-paid role will manage schedules, oversee patient care, and recruit compassionate caregivers to support clients at home.

You will monitor daily services, ensure visits are logged for billing and EVV compliance, and build strong relationships with referral sources to grow the patient caseload. Qualifications include strong communication, two years in healthcare, and proficiency with

As the Patient Care Coordinator , you will be responsible for monitoring and supporting the daily activities of services being provided to clients in accordance with state regulations and company policy, while ensuring quality coordination of service for our patients.
