Job summary

HomeWell Care Services is seeking a compassionate Care Coordinator to join our Duval/Nassau/Clay Counties team in Jacksonville. This part-time role offers real full-time potential as we grow, working closely with Agency Directors in a supportive, fast-moving environment where your work makes a difference daily.

You will manage caregiver scheduling, maintain client records in WellSky, support onboarding and compliance, and act as a main contact for families and field staff.

Qualifications

• Experience in home care or home health as caregiver, scheduler, or coordinator.
• Active HHA or CNA certification.
• Comfortable stepping into the field when needed.
• Familiarity with WellSky is a plus but not required.
• Proficient in Microsoft 365 including Word, Outlook, and OneDrive.
• Warm, professional presence with strong people skills and compassion.
• Organized, reliable, and able to juggle multiple priorities.
• Valid Florida driver's license and reliable transportation.
• Must pass a Level 2 background screening.

Responsibilities

• Be the go-to person for caregivers and families, keeping communication clear and care consistent
• Manage scheduling and step in to cover shifts in the field when needed
• Keep client records and care plans current and accurate in WellSky
• Support onboarding, compliance tracking, and day-to-day operations
• Flag concerns to our supervising RN and Agency Directors and help find solutions fast
