Job summary

Right at Home is seeking a dedicated Care Coordinator in Colorado to coordinate client care, manage caregiver schedules, and maintain strong relationships with families and partners. The role includes participation in an on-call rotation with evening, weekend, and holiday coverage to ensure uninterrupted client care.

The position emphasizes communication, organization, and teamwork, with opportunities to engage in community events and recruitment support.

Qualifications

• Previous experience in care coordination, healthcare, home care, staffing, or a similar role is preferred.
• Strong organizational and time management skills with exceptional attention to detail.
• Ability to multitask and remain calm under pressure in a fast-paced environment.
• Excellent verbal and written communication skills.
• Strong customer service and problem-solving abilities.
• Ability to work independently while collaborating effectively with a team.
• Must be willing to participate in an on-call rotation, including evenings, weekends, and holidays.
• Must be willing to attend occasional weekend and evening community events and marketing activities as needed.
• Experience with WellSky or similar home care software is preferred.
• A compassionate, dependable, and professional demeanor.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate ongoing care for assigned client caseload, ensuring supervisory visits and plan of care updates are completed on time and communication with clients and caregivers is maintained.
• Manage and adjust schedules to accommodate changes in client needs, caregiver availability, call-offs, and emergency staffing situations.
• Communicate professionally with clients, families, caregivers, referral partners, and office staff regarding scheduling and care needs.
• Participate in the office on-call rotation, responding promptly to after-hours staffing emergencies with additional compensation for on-call shifts.
• Assist with finding coverage for open shifts, including evenings, weekends, holidays, and last-minute staffing needs.
• Support recruitment, onboarding, and caregiver retention efforts as needed.
• Represent Right at Home at community outreach events, health fairs, networking opportunities, and educational events, including occasional evenings and weekends.
• Ensure compliance with company policies, procedures, and all applicable regulatory standards.
• Perform other administrative and coordination duties as assigned.
