Job summary

Right at Home - Northern Colorado is seeking a dedicated Care Coordinator to coordinate care for clients, maintain relationships with caregivers, and ensure continuity of care. This fast-paced role requires strong organizational skills, communication, and the ability to respond quickly to staffing needs.

It includes participation in an on-call rotation with additional compensation and occasional weekend events.

Qualifications

• Experience in care coordination, healthcare, or home care preferred.
• Strong organizational and time-management abilities.
• Excellent verbal and written communication skills.
• Ability to multitask and stay calm in a fast-paced environment.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate ongoing caregiver and client care and update plans on time.
• Manage schedules to accommodate changes and emergencies.
• Communicate scheduling and care needs with clients, families, and staff.
• Participate in after-hours on-call rotation with added compensation.
• Assist with finding coverage for open shifts.
• Support recruitment, onboarding, and caregiver retention.
• Represent Right at Home at community events and outreach.
• Ensure compliance with policies and regulatory standards.
• Perform other administrative and coordination duties as assigned.
