Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Mental Health Care Coordinator to assess needs, develop person-centered recovery plans, and coordinate services for children and families facing mental health challenges.

You will provide skills training, crisis management, and facilitate liaison with service providers to support goal progress and well-being. Strong documentation and compliance are essential in this role.

Qualifications

• Bachelor's degree in social, behavioral, or human services; QMHP-CS credential required or able to credential.
• Maintain CPR and Prevention and Management of Aggressive Behavior training within 3 months of employment.
• Valid Texas driver’s license and auto insurance.

Responsibilities

• Assess needs and develop recovery and life-management plans with individuals.
• Provide relevant skills training to improve daily living.
• Collaborate with service providers and agencies to coordinate care.
• Write and document clearly with correct grammar for valuable records.

Job description

A Mental Health Care Coordinator will assess needs,develop treatment plans, and effectively collaborate with individuals and theirfamily members. The Care Coordinator will assess strengths and needsand, in collaboration with the individual, develop a person-centered recoveryplan. The Care Coordinator will provide skills training, crisisprevention/management, linkage, and referral to support goal progress forchildren and families living with mental health challenges. Thiscompassionate, caring, and supportive team member will contribute to the skillsimprovement, achievements, and overall potential success of a child and theirstate of well-being.

You willget to:

• Assess individual needs andcollaborate with them to develop and execute effective recovery and lifemanagement plans.
• Provide relevant skills trainingto improve daily life.
• Collaborate with serviceproviders and agencies to be a liaison for effective care and positiveoutcomes.
• Write and document articulatelywhile using correct grammar so that documentation is clear and valuable.
• Maintain compliance with alllegal requirements and company policies and procedures.
• Consistently meet withindividuals to maintain progress as well as compliance.

You're definitely need:

• Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
• Maintain a valid Texas state Driver's License and auto insurance.
• To complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment

Preferred -

• At least one year of experience implementing the Child and Adolescent Needs and Strengths Assessment or the Texas Resilience and Recovery Model for Adults or Children.
